Player Registration

Before your child attends their first session with Anssi Jaakkola Coaching, we ask all parents/guardians to complete a short registration form.

This helps us create a safe, well-organised and professional coaching environment for every player.

Why we ask for this information

The form allows us to:

• Understand any medical conditions or allergies

• Be aware of any additional needs or requirements

• Ensure we can respond appropriately in an emergency

• Provide the best possible coaching experience for your child

What happens next

Once you’ve completed the form:

• Your child’s place in the session will be confirmed

• We’ll have everything we need to support them properly

• You’ll receive any final session details if required
